Method & assumptions
Domain score = 25 × sum(weight × answer) ÷ answered applicable weight. Overall score is the equal-weighted domain mean only with at least 80% overall answer coverage and 70% in every domain. Unknowns are not zero.
Hypothetical worked example
With every applicable response at 3 out of 4, each domain and the overall profile score 75 out of 100. All-unknown responses produce an incomplete profile, not a passing score.
